Superbills & Insurance Reimbursement

A Superbill is an extended statement that we can provide on behalf of your provider. It contains all the required information for submitting claims to your insurance company for reimbursement. Coverage varies based on your plan and out-of-network (OON) benefits.

If your insurance reimburses a percentage of the cost per visit, they will typically reimburse you directly. Please check with your insurance provider. If you choose to bill OON, you agree to pay the full fees at the time of your appointment, regardless of insurance reimbursement.

For your convenience, our Office & Billing Manager can check your out-of-network benefits. Most Zeal Care members operate as out-of-network providers and are not contracted with any insurance company.

In-Network Coverage

Some Zeal Care members and private practices are in-network with major insurance providers, making care more accessible and reducing out-of-pocket expenses.

Coverage Details

Session costs, copays, and coinsurance vary by insurance provider and clinician. Not all providers accept all insurance plans. Please contact our office to confirm if your provider is in-network and to verify your benefits. The most accurate coverage information will come directly from your insurance provider.

Good Faith Estimate

Under federal law, you have the right to receive a “Good Faith Estimate” of the cost of services provided. Health care providers are required to give an estimate for patients who do not have insurance or are not using insurance.

You have the right to receive a written Good Faith Estimate at least 1 business day before your scheduled service. This estimate includes costs for medical tests, prescriptions, equipment, and hospital fees.

If your final bill is at least $400 more than your Good Faith Estimate, you have the right to dispute the charge.
